What belongs in context?

Keep context for continuing work and for later historical review: purpose, accepted decisions, constraints, discoveries, evidence, and unresolved questions. Preserve completed decisions and findings even when the next task does not need them, so people and agents can trace the reasoning.

What is a proposed change?

A proposal is a candidate, not accepted truth. Review its changes and reasoning before accepting it. Keep the approval decision, acceptance, and any later release distinct.

What does a handoff preserve?

Preserve the goal, current state, important evidence, and the next action. History explains how the work arrived here; it does not automatically replace current accepted instructions.
